Job Description

Minimum Requirements

  • Master’s Degree in Speech-Language Pathology from an accredited program.
  • Completed Clinical Fellowship Year (CFY) and current Certificate of Clinical Competence from ASHA.
  • Active Ohio state license to practice Speech-Language Pathology is required to start the assignment.
  • BLS certification from AHA or ARC may be required.
  • Must have at least 12 months of Home Health experience within the past 3 years (client requirement).
  • Experience with HCHB (HomeCare HomeBase) and OASIS documentation is required (client requirement).
  • Productivity expectations: approximately 32 productivity points per week for a 40-hour week (client-specific expectation).
  • Preferred: prior experience working with the specified territory models (Blue/Royal/Sienna) or similar caseload organization.


Assignment Details

  • Facility type: Home Health.
  • Shift: Days.
  • Schedule: 8-hour shifts, Monday through Friday.
  • Length of contract: 13 weeks.
  • On-call, weekend, and holiday coverage: none required.
  • EMR: HCHB and OASIS experience required.
  • Territory: Blue/Royal/Sienna coverage model.
  • Positions available: 1.
