The Stroke Coordinator reports to the Assistant Vice President (AVP) of Medicine and Behavioral Health and takes direction from the Stroke Coordinator. The Stroke Coordinator holds a key clinical position within the division. In collaboration with the AVP and Stroke Coordinator, the Storke Coordinator assumes the responsibility of coordinating and articulating the philosophy, objectives, policies and procedures of Albany Med, and the Nursing Service into effective action. The Stroke Coordinator serves as unit-based stroke clinician and acts as liaison to clinical staff, physicians, support staff and others involved in promoting patient outcomes across the continuum.

Job Description

• Educates and navigates patients and their families, explaining the role of the care coordinator, Navigator and Stroke program, providing educational information for all phases of care and expected outcomes including patient/family's role and responsibilities in enhancing their own recovery.

• Monitors delivery of care by completing patient rounds, documenting care, identifying progress towards desired care outcomes; intervening to overcome deviations in the expected plan of care, reviewing the care plan with the patient/family in conjunction with the direct care providers and interacting with involved departments to negotiate and expedite scheduling and completion of test procedures and consults, preventing care delays.

• Oversees patient care by acting as a liaison to the patient care team, participating in interdisciplinary rounds and other meetings; coordinating information and care requirements with other care providers, resolving issues/pathway variances that could affect smooth care progression, fostering peer support and providing education to the multidisciplinary team regarding the hospitalization process.

• Participates in discharge planning assisting with identification and resolution of the barriers to discharge in collaboration with social work, utilization management, home health care, care team and family to facilitate timely discharge.

• Responsible for follow up post discharge telephone calls to patients/families and post discharge provider communication.

• Develops tools to facilitate the Stroke Program including patient/staff education tools, protocols, procedures and guidelines.

• Reviews literature, staying up to date on evidence-based practice of the stroke program.

• Monitors/audits compliance to stroke protocols, provides date to Stroke Coordinator and care team.

• Makes recommendations regarding interventions to improve patient care and outcomes.

Qualifications and Ideal Characteristics

  • Registered nurse with a current license in New York State.

  • Bachelor's degree required. 

  • Master’s degree preferred.

  • Demonstrated clinical competence and knowledge in the specialty area.

  • Demonstrated effective communication skills both written and verbal.

  • 5-7  years of Stroke, Neurology or Critical Care experience required
