Job Details
Seeking a Home Health Occupational Therapist to provide in-home care to adult and geriatric patients across Manhattan.
What You’ll Do:
Evaluate and assess patient functional status in the home
Develop and implement individualized treatment plans
Deliver occupational therapy services focused on mobility, ADLs, and independence
Collaborate with physicians and interdisciplinary care teams
Educate patients and families on treatment plans and safety
Complete timely and accurate documentation
Ideal Background:
Licensed Occupational Therapist (NY)
2+ years of experience (home health or clinical)
Strong independence and field-based experience preferred
