Contract DetailsLocation: Zanesville, OH 43702Employment Type: Per DiemSetting: Home Health / In-Home VisitsSchedule: Flexible, visit-basedPay Rate: $28.00 per hourMileage Reimbursement: Provided for eligible travel between visitsStart Date: ASAPEmployment Status: W2 Employee
Requirements• Active Ohio or compact LPN license• Home care or visit-based nursing experience preferred• Ability to work independently in a home setting• Valid driver's license and reliable transportation required• Ability to pass background screening requirements• Professional and compassionate patient care approach
Responsibilities• Perform in-home nursing visits per established plan of care• Administer medications and treatments as ordered• Conduct patient assessments and monitor changes in condition• Perform wound care and other skilled nursing tasks• Educate patients and families on care plans and medications• Maintain accurate and timely documentation
