Minimum Requirements

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Current Physical Therapist license in Connecticut required prior to the start of assignment.
• Current CPR certification required.
• Valid driver’s license, current vehicle insurance, and access to a dependable vehicle for travel to patient homes.
• Experience providing physical therapy in a Home Health setting preferred.
• Experience with patient assessment, development of plans of care, and documentation in home health workflows preferred.
