Job Description

MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

 

Quality Improvement Specialist (LPN Required)

📍 Travel to sites across Kentucky
💼 Full-Time | $60,000–$65,000 per year + mileage reimbursement
🕘 Monday–Friday, Day Shift with occasional weekends.

About the Role

Are you a detail-oriented Licensed Practical Nurse (LPN) or Registered Nurse (RN) with a passion for quality and compliance? Join our team as a Quality Improvement Specialist, where you’ll play a key role in ensuring high standards of care across our Kentucky centers. This is a dynamic, travel-based role where no two days are the same, and your work directly impacts the lives of the individuals we serve.

What You’ll Do

  • Provide technical assistance and clinical expertise to support quality improvement initiatives
  • Conduct medical record audits to ensure compliance with state and company standards
  • Review, file, and maintain accurate and organized medical records
  • Perform data entry and documentation within the medical records system
  • Travel between centers to support audits, training, and quality initiatives
  • Collaborate with site teams to identify areas for improvement and implement solutions

What We’re Looking For

  • Active LPN or RN license (required)
  • 3+ years of experience in quality improvement, healthcare, or a related field
  • Strong knowledge of medical documentation and compliance standards
  • Excellent organizational skills with strong attention to detail
  • Ability to manage multiple priorities in a fast-paced environment
  • Strong communication and interpersonal skills
  • Valid driver’s license, reliable vehicle, and auto insurance (travel required)
  • Passion for delivering high-quality, person-centered care
