Minimum Requirements

  • Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
  • Current Physical Therapy license in Texas required prior to the start of the assignment.
  • Current CPR certification required.
  • Valid driver’s license, current vehicle insurance, and reliable access to a vehicle for patient home visits.
  • Experience in Home Health and experience with HomeCare HomeBase (HCHB) required.
  • Ability to evaluate functional status, develop plans of care, document per CMS requirements, and complete progress notes and discharge summaries within agency timelines.
  • Willingness to participate in weekend rotation and be available for observed holidays as needed.
  • May be required to supervise Home Health Aides and PTAs per state regulation and agency policy.
  • Preferred: Previous travel Physical Therapy experience. (Preferred items are optional and not required.)

  • Schedule requires flexibility to meet patient needs, including participation in a weekend rotation and potential holiday availability.
  • Visits occur across the agency service area and may include coverage of nearby agency locations as needed (floating to other sites).
