Minimum Requirements

  • Associate, Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Florida Physical Therapist license required to start the assignment.
  • Home health care experience required. Experience providing in-home patient evaluations, treatment planning, and documentation is required.
  • Experience with HCHB EMR preferred.
  • BLS certification from AHA or ARC may be required.
  • Ability to travel within and between service counties (Indian River County, St. Lucie County, Martin County) during a shift.


Assignment Details

  • Facility type: Home Health.
  • Shift name: Days.
  • Shift hours: 8:00 AM to 5:00 PM.
  • Work days: Monday through Friday.
  • Weekly hours: Guarantee 32 to 40 hours per week.
  • Coverage areas: Indian River County, St. Lucie County, Martin County. Travel between patient homes required.
  • Productivity expectations: 30 points.
  • On-call and weekend requirement: None.
  • EMR: HCHB experience preferred.
